Latest information for UltraMED changeover to Palmetto

 

Palmetto GBA has been selected by the Centers for Medicare and Medicaid Services (CMS) to serve as the new A/B Medicare
Administrative Contractor for Jurisdiction 1 (J1), which includes California. The final date for us is 9/2/08, but you must take action sooner.

In order to make the process easier, we are providing the information needed here on our web site. If you are sending your Medicare claims
to Office Ally then follow their instructions! We have provided the form fill with our information all ready filled in.
Step one is to download and fill out the "Jurisdiction 1 Electronic Data Interchange Application".

This is a PDF which can be filled out and printed. We have filled the part of the form which might give you problems. The rest should be easy.
Do this as soon as possible and mail. We will be providing the next set of instructions by July 28th at which time you should be able to send your first
claims to Palmetto.
